ODS Individual HSA Plans

• The HSA PPO Plan is an Health Savings Account (HSA) qualified plan. This plan is a high deductible health plan that waives your deductible for preventative care. After enrolling in the ODS HSA PPO Plan you will be eligable to sign up for an HSA. This high deductible plan is designed to give individuals and families greater control of their healthcare services and choices. The plan has a $2,000,000 lifetime maximum. • The HSA Choice Plan is an Health Savings Account (HSA) qualified plan. This plan is a high deductible health plan that waives your deductible for preventative care. After enrolling in the ODS HSA Choice Plan you will be eligable to sign up for an HSA. This high deductible plan is designed to give individuals and families greater control of their healthcare services and choices. The plan has a $2,000,000 lifetime maximum. Underwriting
Applicants must be approved by ODS underwriting. Effective dates are assigned by underwriting and are generally the first day of the month following underwriting approval.

Billing
Members have the option to pay monthly by electronic checking account deduction, or quarterly via a billing statement. Premium must be collected and sent in with the application.
